United Fit Participation Consent Form, Waiver, and Indemnity Agreement

The facility and instruction offered by United Fit has been established to provide an optimum level of exercise benefit without compromising the health and safety of those who utilize the United Fit facility and/or participate in United Fit activities. I understand the inherent risk of any exercise activity and equipment/facility usage at United Fit. Due to the nature of any exercise program and equipment usage not excluding United Fit training methods, I understand that damages, injuries, or even death may occur without proper usage of facility equipment and unsupervised physical activity.

By purchasing the United Fit Membership and submitting my information for myself and each sponsored dependent, I assume full responsibility for death, injuries or damages which may occur at United Fit to myself or sponsored dependents in, on, or about the premises of United Fit. I hereby fully and forever release and discharge United Fit coaches, volunteers, representatives, agents, and staff from any and all suits, claims, damages, costs and expenses of every kind, in conjunction with the use of United Fit equipment and instruction. Except that arising out of gross negligence or willful misconduct of United Fit staff. I further understand that I am not permitted to bring non-members to the United Fit facility without prior written approval of United Fit employees, representatives, agents and/or staff.

By purchasing the United Fit Membership and submitting my information for myself and each sponsored dependent, I agree to indemnity and hold harmless United Fit employees, subtenants, volunteers, representatives, agents, and staff from any and all suits, claims, damages, costs and expenses of every kind, in conjunction with the facility and equipment United Fit provides during gym hours. The undersigned for me and each sponsored dependent further agree to use the facility, all the equipment, lockers and exercise areas properly and leave in good condition, and assume total liability for all bodily, facility or equipment damages incurred through the misuse of any facility area and/or equipment during gym hours thereof. I also understand that United Fit employees, subtenants, volunteers, representatives, agents, and staff are not responsible for any lost/stolen personal property, and that items will be removed from lockers if left longer than (1) day, and non-returnable without a valid driver’s license/ID, and thorough description of removed item(s).

By purchasing the United Fit Membership and submitting my information for myself and each sponsored dependent, I agree that I may desire to voluntarily engage in an exercise program provided by and/or instructed by United Fit staff to improve health and fitness. I understand medical clearance is recommended before beginning an exercise program. Consultation with my physician to gain clearance to begin a fitness program is my responsibility, both from myself and for my sponsored dependents, and is highly recommended.

By purchasing the United Fit Membership and submitting my information for myself and each sponsored dependent, I certify that I am a consenting adult of legal age and the information I have given is correct and accurate. I have provided complete and current contact information. I agree that in the event of an emergency where I cannot be reached, emergency medical treatment may be provided to my sponsored dependent. By my signature below, I agree to provisions of the United Fit Participation Consent Form, Waiver, and Indemnity Agreement for myself and each sponsored dependent, guest, and their respective heirs, and assigned, intending to be legally bound by this agreement.

Our Outdoor Gym known as MUSCLE PARK is Albuquerque's 1st Outdoor Workout Gym outfitted with over 1000sq. ft. of turf, cardio equipment, free-weights, Deadlift Platform, Leg Press, Hack Squat, Leg Curl, Leg Extension/Curl Combo, Two Olympic Bench Presses, Olympic Military Press, Lat Pulldown, Mid-Row, Pullup Dip station, Yoke, Atlas Stones, Sleds, Tires and Sledge Hammers.
